I need a quote on the following work for my website

a. Insert phone number and email link in big on top visible at all times on ALL pages
b. Make an analysis of the website’s compliance and Google best practices. Also SEO general best practices including traffic increase and conversion rate.
c. Implement all the items on the Google Website best practices checklist listed in the Webmaster Guidelines here
d. Create a small 4-5 filed Front page estimate request form
e. Create an additional page called “cash for leads” containing a form that people would fill out if they have a lead to forward to me in exchange for cash. I can give more details on this later.
f. Create a Site index
g. Create an area of the site where clients and prospective clients can go to shop for materials, pull out forms to help them identify materials, look at articles and pictures to help them make a decisions about their renovation.
h. Help with creation of ad that is known and proven to maximizes CTR in the remodeling industry
